Herniated Disc

Here are a couple of pictures you don't want your doctor to show you... The side view in these MRI images shows that the disc is bulging or herniated. What we want to discuss here is, what does that mean exactly???


Let me offer you an analogy I often share with my patients. Imagine that the disc between the vertebra looks like a thick slice of onion (see the layers) and in the middle of the onion about a 1 inch spot is popped out and replaced with a balloon filled with some toothpaste. The toothpaste represents the nucleus pulposus and the onion represents the anulus fibrosus. 

The picture above implies that there are 3 ways a disc can look. Nothing could be further from the truth! In reality your discs can look many different ways but the general thing to take away is that the toothpaste like nucleus can tear through the cartilage like anulus causing it to swell and putting pressure on the nerve as it exits through the small hole called the neural foramina. There is a final stage not shown that involves the nucleus actually rupturing. See below for that.

So, the above picture is a sequestered disc and more often than not it will require surgery to remove the sequestered segment of the disc.
